Good Article
I want to know how I should prepare for upsc cms exam. .whether I shud take coaching classes or self study.. kindly suggest Dr rekha mam
Hi doctor.
It will be more easy for me if know your present educational state (year of mbbs).
In general, Post interns who are preparing for neet pg/aiims they find upsc cms a bit easy to crack as it contains partial syllabus (Preventive medicine+general medicine+pediatrics+general surgery+OBG). Though the questions look simple, most of them are tricky and conceptual. You will realize it if you solve last year question papers.
We have published free solved upsc cms papers in our site. Go through them thoroughly.
There are no coaching institutes as of now which are exclusively offering those five subjects. UPSC MEDICO is planning to start a batch in 2022.
If you are exclusively preparing for upsc cms and not into neet pg coaching, you need to go through question banks specific to those five subjects mentioned above. Repeat percentage is high, so do solve all old papers (last five years).
All the best

Sir I was a neetug aspirant 2022 I am getting desent marks to get into government medical college I want to give upsccse exam along with completion of my internship. But I am confused how to deal with NExt exam along with upsc preparation if I take medical sciences as my optional. Please do guide us. I have a lot of time after neetug 2022 & joining mbbs what effective utilisation I can do now in this regard.
Please do help the early starters
It’s too early to be thinking in lines of the UPSC exam, 6 years ahead. For now, focus on mbbs basics in 1st and 2nd MBBS, then begin your medical sciences optional preparation from 3rd MBBS. It doesnt make any difference, if it is NEET PG or NEXT (Qualifying), at the end of the day you need to build medical concepts to crack these (including Board Theory exams). So, enjoy the process of learning medicine. There is an unneccessary panic created by coaching institutes in the name of NEXT, to fill up foundation batches starting from FIRST MBBS (It’s their marketing strategy) DONT FALL PREY FOR IT. With regards to UPSC General studies preparation, start making a habit of daily news paper reading. And also work on commmunication skills, participate actively in college extracurricular activities, learn administrative skills, develop empathy towards society, debate and question yourself. UPCS exam is in a way is a test of the personality too. Make wise use of available time. ALL THE BEST.

Sir typhoid has longitudinal or transverse ulcer ??? And what are the type of ulcers in tuberculosis?
CHARACTERISTICS OF INTESTINAL ULCERS IN VARIOUS INFECTIONS
Oval and Longitudinal in Typhoid
Transverse ulcers in Tubeculosis
Flask shaped ulcers in E.hystolytica

All the best from Team upsc medico
what is shigellosis
does shigellosis include all diseases caused by all serogroups of shigella
are shigella food poisoning and bacillary dysentry synonymous
Shigellosis is a diarrheal disease caused by a group of bacteria called Shigella. There are four different species of Shigella:
Shigella sonnei (the most common species in the United States)
Shigella flexneri
Shigella boydii
Shigella dysenteriae
Dysentery caused by shigella is synonymous with bacillary dysentery.
good evening sir,
for an answer to management of dengue, should we write as management of DHF grades 1,2,3,4 and compensated shock an profound shock as given in park or dengue with warning signs and severe dengue as explained in video
According to the syllabus, the topic dengue is in General Medicine, not is PSM. Thats the reason behind the clinical approach (From OP GHAI and Harrison) instead of community-based approach in my video. But frankly speaking, a community-based approach to the management of dengue will fetch you more marks in the exam, as the trend is more towards preventive approach and strategies for clinical ailments. Thanks for asking this question, I ‘ll be uploading community-based approach management video as per NVBDCP regulations.

Sir, in factors regulating CO the last part you told due to decrease in extra cellular Na+ results in decrease in Na/Ca exchanger but it should be reverse right?
Outgoing NA+ chooses NA+/CA+2 exchanger in inhibition of its normal NA+/K+ ATP ase,,,,which makes CA+2 to move into the cells in exchange of NA+…..more CA–more contractlity>>>CA CHANNEL BLOCKERS to decrease STROKE VOLUME!!! I hope you got it!!!
I guess, Decreased extracellular sodium. Will lead more efflux of sodium and simultaneously more influx of calcium. Leading to increase of contractility..
How should I send you the copy I wrote, where is upload link

Sir as q asked in 2017 in contact dermatitis
A q asked about mason
Sir is it allergic or irritant dermatitis
I think this is allergic
But you tell irritant
It’s a 2014 question, and the diagnosis is Irritant contact dermatitis. Cement material is an irritant as per FITZPATRICS. Any irritant contact dermatitis might end up as allergic dermatitis in the long run. Even if u mention diagnosis as Alleric CD (you will not loose marks). I will soon upload another video differentiating ACD vs ICD.

Medical sciences optional is a mix of multiple subjects (11), If you choose a subject of your expertise, better to spend minimal time, weaker subjects and clinical subjects might require more time allotment. Strategy and schedule are purely personalized, from institute end-it’s not possible to give a ready made schedule as such. For personalized assistance feel free to contact 8978444188. You can find strategy video for 2024, in upsc medico you tube channel. Might help you, kindly explore.

Role of Vitamin C in cholesterol metabolism (TIME STAMP-08:14)
Ascorbic acid is involved in the regulation of cholesterol metabolism in several ways.
Dietary inadequacy of vitamin C is associated indirectly with a lowering of cholesterol absorption, this effect resulting from a reduction in the availability of bile acids, monoglycerides and fatty acids.
Cholesterol synthesis is decreased in vitamin C deficiency due to a decrease in the activity of the microsomal enzyme cholesterol 7 alpha-hydroxylase.
Role of Vitamin B3 in cholesterol metabolism
Nicotinic acid (Niacin) when given in higher doses reduces plasma lipids.
Nicotinic acid reduces production of VLDL in liver by inhibiting TG synthesis.
It inhibits intracellular lipolysis in adipose tissue and increases the activity of lipoprotein lipase that clears TG.
It Is the most effective drug to raise HDL-CH by decreasing rate of HDL destruction
Dose limiting side effect is marked flushing, heat and itching (cutaneous vasodilator)

Sir right ventricular blood play any role in cardiac output?
Cardiac output (CO) as we clinically measure it refers to left ventricular output into the systemic circulation. So the blood inside the RV is not part of systemic cardiac output. But RV function is crucial because its stroke volume determines LV preload. Therefore, RV output indirectly but critically maintains cardiac output.

Sir,why does dpp4 inhibitors don’t cause weight gain?they also increease glp right
Good question: DPP-4 inhibitors raise endogenous GLP-1 only to physiological levels, causing glucose-dependent insulin secretion without significant appetite suppression or insulin excess → hence weight neutral.

Sir there are gina 2025 guidlines still we have to learn the old ones
Old explanation still works as there are no major updates: quoting here the gist of GINA 2025 guidelines for reference:
The transition from GINA 2024 to GINA 2025 asthma guidelines reflects a clear shift toward safer, more personalized, and anti-inflammatory–focused care. While GINA 2024 had already introduced the preference for ICS–formoterol as a reliever (Track 1) over SABA, GINA 2025 strengthens this further by promoting ICS–formoterol across almost all treatment steps, significantly de-emphasizing SABA due to safety concerns. A key conceptual change is the discouragement of the term “mild asthma,” highlighting that even seemingly mild disease carries a risk of severe exacerbations or death, thereby reinforcing early and consistent use of inhaled corticosteroids. The 2025 update also places greater emphasis on objective diagnosis, even in patients already on treatment, and expands the role of biomarkers like FeNO and blood eosinophils in diagnosis, phenotyping, and guiding biologic therapy. In addition, there is a stronger push toward individualized treatment and shared decision-making, including inhaler choice based on patient preference and environmental considerations. Pediatric asthma (especially ≤5 years) now has clearer diagnostic criteria, and updates in exacerbation management aim to avoid overtreatment. Overall, GINA 2025 advances toward precision medicine, with less reliance on symptom-based labels and more focus on risk reduction, inflammation control, and tailored therapy.
